A form, a policy or other document used to write insurance, which has been adopted and is used by a large number of companies or which has been promulgated by a rating bureau or legislature. The use of standard forms does away with much of the need to scrutinize every word of a policy form for meaning, since the standard has been examined and adjudicated by courts.
